ADMISSION PROCESS
We are very excited that you have made the decision to join our School Family. There are a few forms that we need to get completed in order to register your family with us. Below you will find links to our enrollment packet, food program forms and then forms that are specific to various age groups.
Once we have received your completed registration forms we will reach out to you to schedule your enrollment interview. We absolutely love sitting down with you to get to know both your child and the people involved in their life. During the enrollment interview we will cover your child’s needs, your needs, our Parent Handbook, our policies, show you thru the check-in/out process, introduce you to their teachers and make sure your account is set up and ready for your child’s first day.
NEW ENROLLMENT
Enrollment Forms
Each child will need their own enrollment forms completed. This form is for either of our locations.
Food Program Forms
Young Sprouts participates in the Child and Adult Care Food Program. We only require one of these per family as it will cover all enrolled children.
Health Statement Form
This is a required for for each child that is not enrolled in public school. This form will need to be downloaded and sent to your child's regular pediatrician or family doctor for a signature. Please return this form to the center or fax it to 817-841-1070. Make sure that the doctor has signed the form and include a copy of the child's shot records.
Food Allergy Emergency Plan
Please complete this form only if your child has an allergy to any type of food that could possibly be served at our center. This form must be signed by your child's doctor.
INFANTS
Infant Care Instruction Sheet
This form must be completed by the parent for each infant under the age of 18 months. This form must be updated every 30 days by the parent either in person or by using this link.
Infant Declaration Form
This form must be completed by the parent for each infant under the age of 12 months.

Our Parent Handbook is downloadable here. Please take a moment to read through the handbook so you understand the policies of Young Sprouts and know what to expect from us and what our expectations of our families are as well.
REQUIRED APP
Each parent is required to download and use the Child Pilot Parent app to their electronic device. This is our main method of communication with our families while you are away from the center.
